Before Your Appointment:
What to bring:
- Bring your Medicare card, DVA gold card, and any private health insurance information.
- Bring your referral letter. You should already have sent this through to book your appointment but it’s good to have a paper backup. We need it before your appointment. If you can’t send it, ask your doctor’s office to send it for you. This letter helps you get some money back from Medicare.
- Bring a list of your medicines or the actual bottles, patches, or inhalers. This helps us know what you take, even if it’s not all the time or if it’s over-the-counter medicine.
- Bring any important blood tests, imaging reports, and scans. If you don’t have a copy of your blood results or scan – please note down the name of the pathology provider or radiology practice NOT the name of the medical centre or GP practice you attended. This helps Dr. Kamalaraj to chase your results for you.
- Bring a friend or family member. They can help you remember things we talk about during your visit.
What to wear:
Wear comfy clothes that are easy to move in, like a short-sleeved shirt and shorts. You might need to undress down to your underwear so we can check the parts of your body that hurt.
Getting here:
We have two locations: one in Gledswood Hills and one in Hurstville. Check our Contact Us page for directions on driving and taking public transportation.
At Your Appointment:
Please leave plenty of time to arrive at our practice and come at least ten minutes early. You’ll need to fill out a form before we see you. The first appointment can last 30-45 minutes, depending on your problems. You’ll need to pay with a credit or debit card (no cash or checks). Please ring us to discuss fees.
After Your Appointment:
- Write down everything you can remember right after your visit so you don’t forget.
- If you think of more questions later, write them down and bring them to your next visit, or ask your regular doctor.
- Keep any forms and prescriptions we give you during your visit safe. Fees may be incurred for replacement forms/scripts.
